By using the MYEASYDOC Website ( MYEASYDOC.COM ) you accept the following terms of service.
Telehealth/Telemedicine involves the use of electronic communications and media to exchange information and to enable health care providers at different locations to share individual patient medical information for the purpose of providing and improving patient care. Providers may include, but are not limited to, MYEASY DOC LLC (including, without limitation, its employees, representatives, members, principals, officers and directors, and including through one of more of its media and/or platforms (i.e., including without limitation any website and/or mobile application), collectively, “MYEASYDOC”), primary care practitioners, specialists and/or subspecialists, nurse practitioners, registered nurses, medical assistants, pharmacists and other healthcare providers who are part of a clinical care team. In addition to the members of your clinical care team, your family members, caregivers, and/or other legal representatives and/or guardians may join and participate on the telehealth/telemedicine service, and you agree (i) to share your personal and medical information with such Providers, family members, caregivers, legal representatives or guardians and (ii) to allow each and every of the foregoing to share such personal and medical information with one another. The information may be used for diagnosis, therapy, follow-up and/or education.
Telehealth/Telemedicine requires transmission, via Internet, tele-communication device or other similar means, of health information, which may include, without limitation:
· Patient medical records
· Medical images
· Progressreports,assessments,orotherintervention-related documents
· Bio-physiologicaldatatransmitted electronically and output from medical devises
· Live and/or recorded videos,pictures,textmessages,audioandanydigitalformof data
The laws that protect the privacy and confidentiality of health and care information also apply to telehealth/telemedicine. Information obtained during telehealth/telemedicine that identifies you will not be given to anyone without your consent except for the purposes of treatment, education, billing and healthcare operations. By agreeing to use the telehealth/telemedicine services, you are consenting to MYEASYDOC’s sharing MYEASYDOCobtaining,
As with any Internet-based communication, you understand that there is a risk of security breach. You further understand that your providers cannot control your environment, surroundings, data connection or equipment. Electronic systems used will incorporate network and software security protocols to protect the confidentiality of patient identification and imaging data and will include measures to safeguard the data and to ensure its integrity against intentional corruption.
Individuals other than your clinical care team or consulting providers may also be present and have access to my information for the telehealth/telemedicine session. This is so they can operate or repair the video or audio equipment used. These persons will be advised adhere to applicable privacy and security policies.
Telehealth/telemedicine sessions may not always be possible. Disruptions of signals or problems with the Internet’s infrastructure may cause broadcast and reception problems (e.g., poor picture or sound quality, dropped connections, audio interference) that prevent effective interaction between consulting clinician(s), participant, patient or care team.
You hereby release and hold harmless MYEASYDOCand
You understand and agree that the health information you provide at the time of my telehealth/telemedicine service may be the only source of health information used by the medical professionals during the course of my evaluation and treatment at the time of your telehealth/telemedicine visit, and that such professionals may not have access to my full medical record or information held at MYEASYDOC.
You understand that state laws require you to be within the state of my residence noted above in order for me to be treated through telemedicine. You hereby acknowledge and agree that you, and your use of any services provided by MYEASYDOC (i.e., any telemedicine undertaken through MYEASYDOC),
You understand that you will be given information about test(s), treatments(s) and procedures(s), as applicable, including the benefits, risks, possible problems or complications, and alternate choices for your medical care through the telehealth/telemedicine visit.
You understand that as with any medical procedure, there are potential risks associated with the use of telemedicine. These risks include, but may not be limited to:
• Information transmitted may not be sufficient (e.g., poor resolution of images) to allow for appropriate medical decision making by the physician and consultant(s);
• Delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment;
• Security protocols could fail, causing a breach of privacy of personal medical information; and
• A lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors.
You have the right to withhold or withdraw consent to the use of telehealth/telemedicine services at any time and revert back to traditional in-person clinic services. You understand that if you withdraw my consent for telehealth/telemedicine, it will not affect any future services or care benefits to which you are entitled.
You hereby accept all risks, including, without limitation, to my health, including disease, injury or death that may result from my use of MYEASYDOC’s services and you hereby release MYEASYDOC, on your behalf and on behalf of your personal representatives, estate, heirs, next of kin, and assigns, from any and all losses, costs, claims, causes of action and damages arising from any and all illness or injury to my person and/or property and/or privacy, including my death, that may result from or occur as a result of my use of MYEASYDOC’s services, whether caused by negligence or otherwise.
You hereby consent to the use of telehealth/telemedicine in the provision of careand the above terms and conditions.
